Continuing Education Enrollment Request

The Continuing Education department may request enrollments in Continuing Education shells by completing the form below. If you need to enroll more than a single user, please select the Multi User Enrollment option to upload a file. Please be sure to include all of the required information including: CSU ID (if applicable), Full Name, Email Address, Continuing Education Shell Name and Role You Want the User(s) to Have.
